HMCD is the local agency that regulates the construction or upgrading of hazardous materials storage/handling facilities in the following geographic areas: within the Cities of Los Altos, Los Altos Hills, Monte Sereno, Palo Alto (underground storage tank systems only), and Saratoga; Moffett Field; San Martin; Stanford; and unincorporated areas of Santa Clara County. In addition, HMCD handles County owned/operated facilities regardless of location, and State, Federal, and Santa Clara Valley Transportation Authority (VTA) facilities within the City of San Jose. HMCD-109	Temporary Fuel Tank Permit Application		This form must be submitted to and approved by HMCD prior to bringing a temporary fuel tank (e.g., compressor, portable standby generator, etc.) on-site at facilities located in Los Altos Hills, Monte Sereno, Saratoga, or unincorporated areas of Santa Clara County (e.g., San Martin, Stanford, Moffett Field).
HMCD-019	Toxic Gas Installation Application		This form must be completed and accompany plan submittals associated with the construction/upgrade of facilities/areas regulated by TGO.
HMCD-020	Toxic Gas Ordinance Field Guide/Checklist		This form should be completed by contractors or facility owners/operators prior to scheduling TGO inspections.

UST-C	UPCF Underground Storage Tank - Certification of Installation / Modification		This form must be submitted along with the UPCF Facility Information: Business Activities, UPCF Facility Information: Business Owner/Operator Identification, UPCF Underground Storage Tank Operating Permit Application - Facility Information, and UPCF Underground Storage Tank Operating Permit Application - Tank Information forms upon the completion of installation or upgrading of tanks and/or piping associated with a UST system.
